Community Health Worker Resources

In 2021, Wisconsin Department of Health Services (DHS) Chronic Disease Prevention Program (CDPP) received funding through the Community Health Workers for COVID Response and Resilience Communities (CCR 2109) grant from the Centers for Disease Control and Prevention (CDC). This guide was informed Community Health Workers that were part of the CCR 2109 Grant to ensure that incoming CHWs all have access to the knowledge and experience of seasoned CHWs in the field.
